CRM Health Navigation
The Fishing Partnership needed to transition from being a health insurance plan to a health navigation company for fishing families. Without having any automated way to support the transition and ongoing navigation services, the Partnership was at risk of not being able to contact, transition and provide ongoing navigation services to a very hard to reach and at risk population.
Working with Partnership leadership and their health navigators, Synthesis successfully helped the Partnership reengineer its processes and deployed CRM for contact management, insurance transition/support and health intervention tracking. Synthesis continues to build out additional capabilities enabling the deployment of more and more services to fishermen and their families.

Ongoing analysis of performance and enhancements within CRM for continuous performance improvement.
With the Installation of CRM at the Partnership, the insurance transition was very successful with virtually every fishing family migrating to an appropriate health plan alternative before the insurance cutoff date. Since then, they have been able to broaden their engagement strategies and has more than tripled its membership base for health navigation. Synthesis continues to build out additional capabilities enabling the deployment of more and more services to fishermen and their families.
